Forms

The Commission has developed standard forms relating the CLAS examination. This will facilitate the efficient management and administration of the CLAS examination.

These forms are in Adobe pdf format and require the Acrobat Reader plugin (available for free download from the Adobe website) to view and print.

They must be printed, filled in correctly and mailed to the address on the form.

  1. Nomination for CLAS examination
    (to be completed when nominating a candidate to sit for the CLAS examination)
  2. Request for information on CLAS examination result
    (to be used when a candidate seeks information on the result of the examination)
  3. Review of examination result
    (to be used to request a re-mark of the CLAS examination)
  4. Request for renewal of CLAS certificate
    (to be used when renewing your certification)
